Anthem’s Medicare Supplement plans are available in five
standardized plan designs: A, B, F, I and J. All plans provide
government-prescribed Basic Benefits, which include:
Basic Benefits
Hospitalization: Part A coinsurance for the 61st through
the 150th day plus 100% coverage for 365 additional days
after Medicare hospital benefits end
Doctor bills and outpatient services: Medicare Part B
coinsurance (the 20% of eligible expenses not paid by
Medicare) after the Part B deductible ($110 in 2005) is
met
Blood: The first three pints of blood each year

Part A deductible ($912 in 2005)
Plan F Benefits

Skilled nursing care coinsurance for up to 100 days

80% of foreign travel emergency costs after $250
deductible for the first 60 days of each trip
Plan I Benefits

Part A deductible ($912 in 2005)
Skilled nursing care coinsurance for up to 100 days
100% of Part B excess charges
80% of foreign travel emergency costs after $250
deductible for the first 60 days of each trip
At-home recovery visits (up to $1,600 a year)
Basic outpatient prescription drugs (up to $1,250 a
year, after $250 deductible)*
Plan J Benefits
Basic Benefits as described above
Part A deductible ($912 in 2005)
Skilled nursing care coinsurance for up to 100 days
Part B deductible ($110 in 2005)
100% of Part B excess charges
80% of foreign travel emergency costs after $250
deductible for the first 60 days of each trip
At-home recovery visits (up to $1,600 a year)
Extended outpatient prescription drugs (up to $3,000 a
year, after $250 deductible)*
Preventive medical care (up to $120 a year)
*Plan I and J members will have their prescription drug
benefits administered by Anthem Prescription Management (APM).
These members will enjoy the following advantages of APM:
Real-time processing, which means no hassle of
collecting receipts and completing a claim form if drugs
are obtained from an APM network pharmacy.
Access to more than 53,000 chain and independent
pharmacies nationwide, including the convenience of
having prescriptions filled at home and while traveling
across the United States.
Anthem’s negotiated discount on the cost of drugs. Plus,
after you reach your Medicare Supplement plan’s
prescription drug maximum, you’ll continue to receive
Anthem’s negotiated discount on the retail cost of your
medication.
Anthem Rx Direct mail service pharmacy, a convenient way
for members to order maintenance medications used to
treat ongoing conditions.
Management of drug utilization to ensure the appropriate
medication is being used for the medical condition and
the proper dosage is dispensed.
